Case Management Program

Grand Gateway’s conflict free case management program serves elderly and disabled individuals in Rogers, Washington, Nowata, Craig, Mayes, Delaware and Ottawa counties in Grand Gateway’s service area. In addition Case Management has been asked to provide services in Tulsa, Creek and Osage counties. Case Management Services promotes in-home community based services that allow participants to remain independent in their own home thus avoiding expensive nursing home placement. These programs may help provide the following services; preparing meals, home delivered meals, bathing, help with housekeeping, help with transportation, incontinence supplies, skilled nursing, counseling, and help with coordinating medical services.

Grand Gateway’s Case Management program provides two programs to individuals in our communities:
  • The Advantage program is designed to help elderly and disabled individuals remain safe and independent in their own homes. To qualify for the Advantage program individuals must be eligible for Medicaid (individual income less than $2,163/month in FY 2013) and be assessed as “Nursing Facility Level of Care” by the Department of Human Services. 
  • The Living Choice evaluation program, check on consumer satisfaction after they have entered the community.
