Family Caregiver Program

Information About Services

We can provide brochures, literature, presentations, just about anything we can to share our services and programs with caregivers! We also have an information booth available for any health fairs or senior days in the communities to help people learn about our services. We have a monthly newsletter which is packed with information and helps the caregiver stay connected.

Below is a list of just some of the information and services we make available for caregivers:

Congregate Meal Sites and Home Delivered Meals 

Noon meals are available at various locations throughout our seven county service area for seniors 60+.  When you cannot get to a nutrition site, meals can often be delivered directly to your home.  Please call for the closest nutrition site or if you qualify for home delivered meals.

Information & Assistance

Outreach workers contact older persons to provide information on services available and to assess individual needs.  Case Management and Information & Assistance is provided on a variety of issues and services.  Linkage and referrals are made as needed.  Please help us, help you!

Legal

Legal assistance is provided by Legal Aid Services of Oklahoma.  This service provides legal advise, counseling and representation by an attorney or other person acting under the supervision of an attorney.  If you need more information about wills, power of attorney, guardianship, debt collection, social security, veteran’s benefits, or more, please call us for referral.

Transportation

Buses and vans provide transportation for older persons to nutrition sites, senior centers, doctors, drug stores, banks, grocery stores, and other important locations to provide for more independent living for seniors.  Transportation is not available in all areas.  Please call us for availability of transportation in your area. 

Long Term Care Ombudsman

Ombudsman Supervisors have information, resources and knowledge to help you find a nursing home for long term or short term placement.  They also monitor and advocate for Quality of Care and Quality of Life of seniors living in Long-Term Care Nursing Homes, Residential Care and Assisted Living Facilities.  Ombudsman Supervisors and Ombudsman Volunteers attempt to resolve complaints from residents, family and friends by exploring problems and recommending corrective action.  Volunteers are trained and certified to spend a minimum of 2 hours per week in a facility.
