What is the Case Management Advantage Program

The Case Management Advantage Program offers a range of services for Medicaid-eligible individuals who need nursing facility-level care but prefer to stay in their own homes or chosen residential settings. This Home and Community-Based program provides an alternative to nursing facility placement, supporting older adults and individuals with disabilities to remain independent while staying close to family and friends. The Advantage Waiver Program emphasizes individual choice and self-direction, ensuring that you receive the necessary care in a setting that suits your personal preferences and needs.
